Your preimplantation embryos will be treated using a protocol designed to destroy embryos, while preserving their DNA. This means that once an embryo’s DNA is preserved, the embryos can NO LONGER BE USED TO BECOME PREGNANT.

The preserved DNA itself cannot be used to bring an embryo back to life. However, it preserves the information about the DNA sequence.

Presently, NO technology exists that can bring embryos back to life using preserved DNA sequence information and it is NOT known whether it will ever be developed.

There is no experience with long-term storage of DNA using this or any other fixation technique, and it is not certain how long or how well the DNA sequence can be retrieved from the embryo’s DNA while it is preserved using the Forever Embryo™ technology.

It is not known how long the stain used to make the DNA visible within the Forever Embryo™ chamber will remain intact.

The embryo DNA sequence preservation technique is a manual process and results in the embryo itself being lost.

The slide containing the preserved embryo’s DNA sequence is made of glass and may shatter. We use the thickest glass slides available to protect the specimen, yet it may break if dropped on a hard surface.

The Forever Embryo chamber contains small parts, including powerful magnets, and must be kept away from children.

Payment and Refund

Payment for DNA sequence preservation is due before service is rendered. There is no refund as long as the DNA sequence from at least one embryo is preserved.

Shipping and Handling

The shipping of Forever Embryo™ time capsule within the United States is included.

The cost of shipping and handling of the embryos for processing to Forever Embryo™ is NOT included and is usually billed by the delivery carrier or by the entity where the embryos are being stored.

The entity where your embryos are located, may charge additional fees for the release and handling of embryos.

Warranty

WE PROVIDE ABSOLUTELY NO WARRANTY related to preserving the embryos DNA, including but not limited to the duration or quality of DNA preservation or the appearance of the DNA after fixation.

Consent

By signing this document, I certify that I understand and accept this agreement in its entirety.

I waive my right to any legal actions against Vitronova LLC for any reasons, and agree that the maximum Vitronova LLC. liability is equal to the amount paid for services under this agreement.

This agreement must be signed by ALL the embryo’s custodians. By failure to disclose all the custodians, the signatory accepts all civil and criminal liability for such action.
